Your plan of care includes 1 on 1 sessions, hands-on manual therapy, Guided exercise progressions, and a custom home exercise program inside our own app.

JAKE SOUVA PT, DPT, CSCS
Hey! I am Jake Souva, The owner of Souva Physical Therapy and Performance! I am a doctor of Physical Therapy and Certified Strength & Conditioning Specialist (CSCS). I graduated from Oakland University in Rochester, Michigan and continue to serve as a teaching assistant in the physical therapy program.
When I’m not working I enjoy spending time with my ever growing family: My wife Nichole, son Brady & chaotic dogs Bubba and Barrett! I’m also a sports fanatic and love playing flag football, basketball, baseball and disc golf (sure we will count it as a sport).
