Jake Souva
PT, DPT, CSCS
Physical Therapist
Strength & Conditioning Coach
I am Jake Souva, The owner of Souva Physical Therapy and Performance! I am a doctor of Physical Therapy and Certified Strength & Conditioning Specialist (CSCS). I graduated from Oakland University in Rochester Hills, Michigan and continue to serve as a teaching assistant in the physical therapy program in the cadaver lab!
I love spending time with my family. My wife Nichole, our sons (Brady and Elijah) and of course our dogs (Bubba and Barrett) bring me joy that I can’t rally put into words. I love everything about sports, whether it be playing, watching, or training for them. My favorite sport is a close contest between football and baseball. My faith in Jesus Christ forms the foundation for everything in my life and I view every day as a way to help others in the way he has called me to.
Dominic Mora
PT, DPT
Physical Therapist
Strength & Conditioning Coach
Hey! I’m Dominic, a Doctor of Physical Therapy and strength and conditioning coach! I graduated from Wayne State University with a strong interest in sports and orthopedic rehabilitation. I enjoy working with patients of all activity levels, from high level competitive athletes to your weekend warrior seeking to move pain free and return to the activities they enjoy. As a lifelong baseball fan, I have a particular passion for working with overhead athletes and helping them return to peak performance safely and confidently. I combine evidence-based practice with individualized, goal-oriented care to support both injury recovery and long-term health. In my free time I enjoy playing baseball, watching Detroit sports, watching movies, and eating great food.

I’ve been going to see Jake at SOUVA For a month now. I went to him for upper back and shoulder pain as a competitive power lifter. Week one of starting physical therapy my mobility was not very great and I only felt 50%. After a few sessions, my whole upper body is feeling back to its 90%. Jake is so kind and he has treated me with nothing but kindness. He gives his full attention to each one of his clients and gives us all the resources we need. Jake gave me an app with my rehab program so even when I’m not at physical therapy I’m still able to do my exercise exercises from home or at the gym! I couldn’t recommend SOUVA and Jake more!Posted on Carrie LittlefieldTrustindex verifies that the original source of the review is Google. Jake is an outstanding physical therapist! He worked with my daughter on her knee injury, and the experience was excellent from start to finish. His knowledge and expertise were clear from day one, and he took the time to explain each step of the process so both my daughter and I understood her treatment plan. I especially appreciated the one-on-one attention he gave her and the consistent communication with us as parents. Jake was patient, encouraging, and genuinely invested in her progress. Thanks to him, she has healed well and feeling confident again. We had a great experience and highly recommend Jake to anyone needing a caring and skilled physical therapist!Posted on John RodriguezTrustindex verifies that the original source of the review is Google. Jake is awesome! He pays attention to what you are looking to get out of Physical Therapy and really helps you reach that goal. I went to him for knee pain after surgery. I first went to another therapist and I did not really see much progress. Jake offers individualized approach to your recovery and only a few sessions in I have noticed improvement! I highly recommend him!Posted on AngelaTrustindex verifies that the original source of the review is Google. Jake is phenomenal. I have a herniated disc at my L5/S1 and he's been incredible to work with over the past several weeks. I've been in between getting surgery and trying to heal on my own, but within the first two weeks working with him, I had already noticed my strength starting to come back and pain decrease - a complete 180 from working with other PT places I've been at. The 1:1 hands on support and knowledge you get is unmatched. I'm a personal trainer myself, so movement is a huge part of my job/life and he's helped build my confidence in re-introducing different movement patterns again. I can't wait to continue to work with Jake on my healing journey!Posted on Leeza FortnerTrustindex verifies that the original source of the review is Google. As an athletic trainer (ATC) who is recovering from an Achilles rupture, I highly recommend Jake Souva. He is filled with knowledge and has helped me achieve more success than I thought I could get at this point in my recovery. Choosing Jake for PT was definitely the right decision.Posted on Sarah HomerTrustindex verifies that the original source of the review is Google. Jake has gone above and beyond helping my son recover from knee injuries!

Because of that, we are only in network with Blue Cross Blue Shield Michigan PPO plans.
If you have an insurance we are out of network with, we will work with you to create a self-pay plan that gets you back to 100% and fits your financial circumstances.

Sports physical therapy is all about helping athletes and active people recover from injuries and get back to performing at their best.
Our approach is designed specifically for athletes, combining sport-specific rehab with injury prevention strategies. Our clinic is located inside of a gym, so heavy duty strength equipment, access to turf for agility and conditioning, and state of the art testing equipment is always available.
Your physical therapy sessions are one-on-one, 45 minutes with a Doctor of Physical Therapy. Plan to arrive 5–10 minutes early so you’re ready to start right on time
Your first visit is a full-body, 1 on 1 assessment. No guessing, just measurable results. We evaluate strength, mobility, movement patterns, and function from head to toe to pinpoint the root of your pain or performance limits. From this objective testing, we build a clear, data-driven plan to get you moving and training at your best, pain-free.
Yes! Every patient gets lifetime access to their custom home program inside the Souva PT mobile app.
Each program has video exercise demonstrations and custom exercise parameters to perfectly fit your needs.
Have a question when you're doing your program? You can message your physical therapist right inside the app.
In most cases, you do not need a physician’s referral to start physical therapy.
You can schedule directly with us and begin your evaluation right away.
Blue Cross Blue Shield insurance does requires a referral, so you will need a script prior to your first session for us to bill insurance.
We’ll guide you through the process to make it simple.
The number of visits depends on the type of injury and complexity of your case. Most non-surgical sports injuries can be resolved in 10–15 sessions. Post-operative rehab, which can take 6–12 months, typically requires a longer plan. We’ll work with you to create a personalized program that fits your goals and your lifestyle.
