jake souva

Jake built this clinic with the vision of providing everything an athlete needs to rehab like a pro. Jake brings experience working with athletes of all skill levels, from recreational to professional. Jake graduated from Oakland University and continues as a teaching assistant in the Doctor of Physical Therapy program.

Although Jake enjoys treating all orthopedic injuries, he has a special interest in ACL rehab and hip/low back rehab. In college, he suffered a low back injury and was told he wouldn’t be able to lift heavy again. After that moment, it became his mission to make sure no lifter or athlete gave up on what they loved doing because of an injury.

His faith in Jesus Christ, his family; wife Nichole, sons Brady and Elijah, and dogs Bubba and Barrett are the foundation his life is built on. 

He is a sports enthusiast, both playing and watching and enjoys TV shows and movies. His favorite sport is football, with baseball being a close second and he loves movies and TV shows.

Dominic Mora

PT, DPT, FDN-1

Dominic is a Sports Physical Therapist and Strength and Conditioning coach who enjoys working with patients of all activity levels, from high level competitive athletes to your weekend warrior. 

As a lifelong baseball player, Dominic has a particular passion for working with overhead athletes and helping them return to peak performance safely and confidently. Whether it is building rotator cuff strength, maximizing core power, or perfecting your pitching mechanics, Dominic will help you become a stronger, more resilient athlete.

Dominic graduated from Wayne State University with a strong interest in sports and orthopedic rehabilitation, combining evidence-based practice with individualized, goal-oriented care to support both injury recovery and long-term health. In Dom’s free time he enjoys playing baseball, watching Detroit sports, watching movies, and eating great food.

Rachael Sweigard

PT, DPT

Rachael is a sports physical therapist who has a special interest in working with active adults, soccer players, and field athletes with a focus on return-to-sport rehabilitation and strength-based performance training.

Rachael played NCAA Division II soccer at Columbus State University in Georgia. She was a 2x All-American soccer player and helped lead her team to an NCAA Final Four appearance. She continues to compete as a professional indoor soccer player and has coached youth soccer teams through the Novi Jaguars. Her passion for soccer has developed into a love for strength training and performance, in her free time she competes as a bodybuilder in the OCB and NPC figure division. Rachael’s athletic background allows her to bring a unique perspective on strength training, recovery and injury prevention to her patients.

Rachael earned her Doctor of Physical Therapy degree from Trine University. Outside of the clinic, she enjoys running local 5Ks, spending time outdoors with her two dogs and fiancé, and trying new coffee and dessert spots in the area.

Because of that, we are only in network with Blue Cross Blue Shield Michigan PPO plans.

If you have an insurance we are out of network with, we will work with you to create a self-pay plan that gets you back to 100% and fits your financial circumstances.

Absolutely! With many patients having high deductible plans now, we are able to accept HSA/FSA cards as payment.

Sports physical therapy is all about helping athletes and active people recover from injuries and get back to performing at their best.

Our approach is designed specifically for athletes, combining sport-specific rehab with injury prevention strategies. Our clinic is located inside of a gym, so heavy duty strength equipment, access to turf for agility and conditioning, and state of the art testing equipment is always available.

 

Your physical therapy sessions are one-on-one, 45 minutes with a Doctor of Physical Therapy. Plan to arrive 5–10 minutes early so you’re ready to start right on time

Your first visit is a full-body, 1 on 1 assessment. No guessing, just measurable results. We evaluate strength, mobility, movement patterns, and function from head to toe to pinpoint the root of your pain or performance limits. From this objective testing, we build a clear, data-driven plan to get you moving and training at your best, pain-free.

In most cases, you do not need a physician’s referral to start physical therapy.

You can schedule directly with us and begin your evaluation right away. 

Blue Cross Blue Shield insurance does requires a referral, so you will need a script prior to your first session for us to bill insurance.

We’ll guide you through the process to make it simple.

The number of visits depends on the type of injury and complexity of your case. Most non-surgical sports injuries can be resolved in 10–15 sessions. Post-operative rehab, which can take 6–12 months, typically requires a longer plan. We’ll work with you to create a personalized program that fits your goals and your lifestyle.
