We believe price transparency matters in healthcare. Here’s how payment works with us.

Being a hybrid clinic means we only accept certain insurances. The reason for this is so we can provide the highest quality care. Currently, we are in-network with BCBS PPO and Out-of-Network with remaining plans.

IIf we don’t accept your insurance, we will explain what you can expect if you still want to work with us

If you do have a BCBS PPO plan, we will bill your insurance as usual. Please note that if you have a deductible, co-pay, or co-insurance, you will still be responsible for a portion or all of the cost of services. If you provide us with your insurance information, we’re happy to verify your benefits and give you a clear estimate of your expected costs.

If you don’t have BCBS, our out-of-network options are simple and transparent. You can expect a comprehensive, high-level evaluation focused on finding the true root cause of your issue. You’ll leave with a clear understanding of what’s going on, exactly how we’ll treat it, and how many sessions it will take to get you back to performing at your best. Pricing for out of network options are below.

your custom plan of care

During your initial visit, your Doctor of Physical Therapy will explain the recommended number of visits needed to help you fully recover. If you choose to move forward with a custom plan of care, we offer pre-payment discounts that can reduce your per-visit cost by  10-20% off the standard session rate.

If you’re unsure whether a custom plan of care is right for you, you also have the option to continue on a session-to-session basis at our single-session rate of $150.
(Optimal results are typically achieved with our custom plan of care option.)*

*Most non-operative patients spend 10-15 visits with us, while post-operative and chronic conditions tend to see us for more visits, often in the 16-24 range.

Absolutely! With many patients having high deductible plans now, we are able to accept HSA/FSA cards as payment.

Sports physical therapy is all about helping athletes and active people recover from injuries and get back to performing at their best.

Our approach is designed specifically for athletes, combining sport-specific rehab with injury prevention strategies. Our clinic is located inside of a gym, so heavy duty strength equipment, access to turf for agility and conditioning, and state of the art testing equipment is always available.

 

Your physical therapy sessions are one-on-one, 45 minutes with a Doctor of Physical Therapy. Plan to arrive 5–10 minutes early so you’re ready to start right on time

Your first visit is a full-body, 1 on 1 assessment. No guessing, just measurable results. We evaluate strength, mobility, movement patterns, and function from head to toe to pinpoint the root of your pain or performance limits. From this objective testing, we build a clear, data-driven plan to get you moving and training at your best, pain-free.

Yes! Every patient gets lifetime access to their custom home program inside the Souva PT mobile app.

Each program has video exercise demonstrations and custom exercise parameters to perfectly fit your needs.

In most cases, you do not need a physician’s referral to start physical therapy.

You can schedule directly with us and begin your evaluation right away. 

Blue Cross Blue Shield insurance does requires a referral, so you will need a script prior to your first session for us to bill insurance.

We’ll guide you through the process to make it simple.

The number of visits depends on the type of injury and complexity of your case. Most non-surgical sports injuries can be resolved in 10–15 sessions. Post-operative rehab, which can take 6–12 months, typically requires a longer plan. We’ll work with you to create a personalized program that fits your goals and your lifestyle.
